Add Name-based Site Note: 1. The values in the input fields are automatically converted to lower-case characters.
2. IP-based plans are available to IP-based sites only, likewise for name-based plans.
3. The default plan is available to both IP-based and name-based sites.
4. Do not prefix www to the site name.
5. To add a site with SquirrelMail enabled, you need approximately 16 MB of free space.
   If MivaMerchant is enabled for the site, an additional 11 MB of free space is required.
